Chronic disease care at Living Assistance Services is overseen by a registered nurse. The nurse builds the care plan, trains and guides the caregivers, and is the first call when a symptom changes. Early detection matters with conditions like heart failure, COPD, and diabetes: catching a change at home often prevents a hospital visit.

Care begins with a free in-home assessment. We meet the client, review medications and current conditions, and build a plan that fits the home routine. How does home care help someone manage a chronic illness?

Home care provides daily support that is hard to sustain alone: consistent medication reminders, help with nutrition and mobility, and a trained caregiver who notices when something is off. With registered-nurse oversight, changes in health are caught early, before they become urgent.

Do your nurses monitor vitals at home?

Yes. Caregivers can take regular readings such as blood pressure, blood sugar, and weight, and report them to the nursing team. The nurse reviews trends, flags concerns, and communicates with the family doctor when needed.

Can home care reduce hospital readmissions?

It can. Many hospital readmissions happen because a change in condition goes unnoticed at home. Consistent caregiver visits and nursing oversight mean symptoms are caught early. We also help clients follow medication schedules and attend follow-up appointments, both of which reduce readmission risk.

How do you coordinate with the family doctor?

Our registered nurse builds the care plan with the medical history and current conditions in mind. When health changes, the nurse contacts the family doctor directly and updates the care plan accordingly. Families receive regular updates as well.
